The Service Catalog is the list of services you sell. Forms &
Templates holds the report forms drivers fill in when they do them. A
service points at one form (its Default Service Report), and that is the
form a driver gets on a job that includes the service.
The Service Catalog
Service Hub → Service Catalog shows each active service with its
category, rate, duration and whether a template is set (Template Set or
"No template"). Search by name, code or description, and switch between grid
and list with the view toggle.
Adding a service
Click Add Service. The Add New Service form has:

Click Create Service (Update Service when editing).
Changing an existing service's category keeps its code so past invoices and
reports still match ("Keep code, change category"); the form then notes that
the code prefix no longer matches the category.
Row actions
Each service's ⋮ menu has Edit, Assign Template (pick or change
its default report form), Duplicate (makes "… (Copy)") and Delete.
Delete asks first and removes the service from the catalog; jobs and records
that already used it keep their history.
Report templates (Forms & Templates)
Service Hub → Forms & Templates lists every report form with its type,
number of sections, version, and a Default or Pre-built label where
it applies. Search, and filter by Type, Source (Pre-built library
or Built by your team) and Sort (newest, recently updated, name, most
sections). Tap a template to view it; its ⋮ menu has Edit Template,
Clone Template and Delete Template.
Every account comes with a library of ready-made forms (septic inspections,
pumping, grease traps, portable-restroom service and more). Use them as they
are, clone and adjust one, or build your own. See

Building or editing a template
New Template opens the builder in two steps, Build and Publish.
Build
(Service, Delivery, Pickup, Inspection, Repair, Septic Service, Event
Service or Other) and Linked Service Categories ("Which service
categories use this template?").

Gallons Captured is the field that feeds per-gallon billing: the number
the driver enters becomes the quantity on any Per Gallon service line on
the same job.
Publish
The Ready to publish summary shows the name, type and section and field
counts. Advanced settings holds Logic (show or hide questions based on
answers), Permissions, Output (how the PDF lays out) and Review (version and
whether it is marked default for its type). Then Publish template, or
Save as draft to finish later.
A draft is not offered anywhere yet: it doesn't appear in a service's
Default Service Report list or in Log Service until you publish it.
Edits and jobs already booked
When a service is added to a job, PortaPro copies its form onto that job. If
you edit the template later, jobs that already have the form keep the version
they were booked with; new jobs get the edited one.
